Capture the event, not just a total

Record what happened, its school or site, location, occurrence time, category and severity.
The occurrence time is separate from the date the event was entered, preserving the context of the event.
Filter the register by status, severity and category to locate the incidents that need review.

An investigation is required before closure

The implemented lifecycle moves from Reported to Under investigation, then Closed.
Closure requires a recorded investigation summary; an incident cannot move directly from Reported to Closed.
Record a root cause when the investigation establishes one. The system does not invent a cause to fill an empty field.

Connect the issue to corrective work

Link relevant findings to the incident within the same organisation and school.
A finding describes the condition; a corrective action records the work needed to address it. These remain separate records with their own progress.
Closing an incident records the investigation outcome. It must not be read as proof that every related corrective action has been completed.

Keep sensitive information in its proper domain

Operational incident records do not provide fields for named pupils, witness statements or medical histories.
Safeguarding concerns use a separate, restricted workflow; ordinary incident access does not grant safeguarding access.
Example: record an unsecured gate and investigate access control without including a child’s confidential case narrative.
