Describe the exposure and its controls

  • Keep the risk, location, owner, existing controls and next review date together.
  • Record inherent and residual likelihood and impact using the identified assessment matrix.
  • Residual risk is an assessor’s judgement; the application does not infer that a listed control has removed the hazard.

Review changes in the real situation

  • Reassess when conditions, equipment or operating arrangements change.
  • Link findings that explain weaknesses in a control, preserving the relationship within the organisation and school.
  • Use review dates and residual exposure to prioritise attention. A coloured score is not an approval to accept the risk.

Example: a delivery route crosses a pedestrian path

  • Record vehicle movement and pedestrian access as the exposure, with the current separation controls.
  • Link a finding when those controls fail in practice.
  • Reassess the residual exposure after corrective work; do not lower the score merely because an action was created.
